Arlight, since I got no help from the help me section it's time to bring out the money, well fake money . Everything I need and want is located in the following post:

You don't have the code the contact form for me, just the CSS, or even find the CSS yourself and steal it, I just need the CSS code! Thank you.

I think you just need to put some kind of scripting condition on the click (with either javascript or php) that triggers the CSS property display:none; on the form. Maybe add visibility:hidden; just in case...

The one that is the easiest one to use is the one. The css can customized such as colors and of course the menu elements can be changed.
